What This Review Looked At

Researchers from Wenzhou University and collaborating institutions in China published a comprehensive review in Biomedicine & Pharmacotherapy examining the potential role of hydrogen therapy in Alzheimer’s disease management. The review synthesised preclinical and early clinical evidence across multiple mechanisms — including oxidative stress, neuroinflammation, amyloid pathology, mitochondrial dysfunction, and cell death pathways — and evaluated the main routes of hydrogen administration currently in use.

Alzheimer’s disease is a progressive neurodegenerative disorder with no disease-modifying treatment currently available. Conventional pharmacological approaches offer limited efficacy and are often accompanied by significant side effects, which has driven growing interest in complementary approaches with favourable safety profiles.

What They Found

Across the reviewed studies, molecular hydrogen demonstrated neuroprotective effects through several distinct pathways relevant to Alzheimer’s progression.

In animal models, hydrogen therapy reduced amyloid-beta accumulation, a hallmark of Alzheimer’s pathology. It also inhibited the NLRP3 inflammasome pathway — a key driver of neuroinflammation — and reduced the expression of pro-inflammatory markers including IL-1β and TNF-α. Microglial polarisation, which governs the brain’s inflammatory response, was modulated in a direction associated with reduced neuronal damage.

At the cellular level, hydrogen protected neurons from oxidative stress-induced DNA and protein damage, reduced cell apoptosis by modulating key regulators including Bax and caspase-3, and supported mitochondrial energy production. The Nrf2 pathway — the body’s primary antioxidant gene expression system — was activated in multiple studies, suggesting hydrogen may strengthen the brain’s own defences rather than simply substituting for them.

Hydrogen’s ability to cross the blood-brain barrier, a significant limitation for many therapeutic compounds, was identified as a key factor in its potential relevance to neurological conditions.

In early human studies, inhalation of hydrogen gas twice daily over six months in patients with Alzheimer’s disease showed evidence of stabilisation and in some cases improvement in cognitive measures. The open-label pilot nature of these studies means they require confirmation in larger controlled trials, but the safety profile across all reviewed studies was consistently favourable, with no serious adverse effects reported.

What This Means in Context

This is a narrative review, which synthesises available evidence without the systematic search methodology of a formal systematic review. The preclinical findings are consistent and mechanistically coherent, but the clinical evidence in humans remains limited in scale. The authors are clear that hydrogen therapy should be understood as a potential complementary approach — not a replacement for standard care — and that larger, well-controlled clinical trials are needed before firm recommendations can be drawn.

The review highlights three main administration routes: hydrogen-rich water, hydrogen gas inhalation, and hydrogen-rich saline. Inhalation was associated with the most direct delivery to the central nervous system.
